Access to clinical records

As a parent or guardian, you can ask to view information from your child's clinical record while on the ward. In some circumstances you may need to provide evidence of your right to access your child's clinical record.

There will be an opportunity to discuss the information in the clinical record and have it explained by a health professional. All access to the clinical record will be under supervision to preserve the integrity of the clinical record.

Requests for copies of information from your child's clinical record must be made to the Release of Information team in the Clinical Record Department. In accordance with the Privacy Act 1993 40 (1), we will respond to your request no later than 20 working days after date of receipt.

Requesting a Copy of a Clinical Record

Information from your child's clinical record can be requested from the Release of Information team in the Clinical Record Department.

The request must be in writing or can be made by completing the appropriate form (Release of Information Request Form). Parents/guardians will need to provide evidence of right to access their child's clinical record.

Please note: If the request is for the clinical record of a patient who is over 16 years old, then consent in writing from the patient is required.
