Paracetamol and Ibuprofen dosing

Paracetamol

Oral

Use in all when enteral route available

<38 weeks 10mg/kg Q8H Max daily <30mg/kg  
Term - 3 months 10mg/kg Q6H Max daily <40mg/kg  
>3 months 20mg/kg Q6H Max daily <90mg/kg Max 4gm/day

PR

Use in children < 25kg when oral route not available

Dosing as per oral doses - round to whole suppositories (25, 50, 125, 250, 500mg)

IV

Use in those > 25kg where oral route not available
Use in those < 25kg when oral and rectal routes unavailable

< 10kg 7.5mg/kg Q6H Max daily 45mg/kg
>10kg 15mg/kg Q6H Max daily 60mg/kg


Loading Doses - These are only for children who have NOT been receiving Paracetamol

PR  40mg/kg Use in children <25kg
PO  20mg/kg Use in children >25kg
IV  15mg/kg Use in children where the PR and PO route is unavailable

Ibuprofen

Caution if using in <1 month of age
DO NOT use in patients with renal impairment

Oral

Load 10mg/kg all ages > 1 month
<3 months 5mg/kg Q8H
>3months 5-10mg/kg Q6-8H
